Your First Visit to an Acupuncture and Chiropractic Care Professional

Upon entering the office, receptionists will welcome you to your new healing facility. You will be asked to complete a patient intake and health history form. This information provides your treatment professionals with vital information about your condition.


Then, you will have a consultation with a chiropractic and acupuncture professional to discuss your health-related issues and concerns. Potential treatment options will also be discussed.
